I, agree to not harm myself in any way, attempt to kill myself during the period counselling,
consulting, psychotherapy, psychometric assessments (the time of my next appointment). I agree that, for any reason,
if the appointed session is postponed, canceled, etc., that this time period is extended until the next direct
meeting with my psychologist/counselor/therapist. In this period of time, I agree to care for myself, to eat well,
and to get enough sleep each night. I agree that these conditions are important, worth doing, and that this is a
contract I am willing to make and keep. By my word and honor, I intend to keep this contract.
Physical violence is extremely harmful to all relationships. Participation in physically aggressive behavior on the
part of one or both partners (or family members) results in numerous negative consequences. Some of these effects
include: loss of trust, loss of respect for self and partner (or family member), emotional and physical pain, lack
of intimacy, and less time spent together. Interpersonal violence greatly interfered with progress in couple therapy.
In fact, interpersonal violence and couple (or family) therapy are not compatible.
For these reasons, we are making a commitment, both written and verbal, to stop all violence in our relationship.
By violence we mean harming or threatening to harm your partner (or family member) either physically (i.e. pushing,
shoving, restraining) or emotionally (i.e. belittling, name calling). I agree that I will not make any violence
during the consulting period at Resilient Minds.
I understand that my treatment records are kept confidential, except where disclosure is required by law or the
ethics of the counseling/psychology profession (i.e. abuse of a child, elderly or disabled person; potential harm or
threat to self or others; child custody cases that go before a court of law; and specific information subpoenaed by
a court of law). The clinical records are the property of Resilient Minds. I waive any right I may otherwise have to
seek to use the clinical records of the Resilient Minds as evidence in any judicial proceedings or to compel the
testimony of any counselor providing counseling to me through the Resilient Minds.
Each sessions last between 45 and 60 minutes. Please remember the importance of keeping your appointment.
